Enrollment

To enroll at Sequoia Jr. High or Sequoia High School, please first submit a Student Interest Form to be sure we have room in your child's grade level.  After you receive notification that you may enroll, please print and complete the following forms and return them to the front office at 1460 S. Horne Mesa, AZ 85204.  If you are unable to print the enrollment forms, you may get them from our from office.  Welcome to Sequoia!
